In epistolary form, its articles were written during the special time of her first year of training to become a general practitioner/family physician, including making the tough decision to give up hospital specialist training, "withdrawing" from fast-paced hospital work, transitioning to working from a metropolitan hospital to a rural community clinic, witnessing and getting used to a younger sibling's growing up, and being independent from a close and loving family. Some also feature a promotion of the author's new academic career at the start of the COVID 19 pandemic and how it was perceived. The book shows a journey of love, strength, perseverance, purpose and encouragement. Lots of anecdotes, lots of stories, lots of quotes. With love and respect.

Yes, I think I am depressed. I am tired, strained and exhausted. Since the last year of medical school, I have lost interest in all things in life. I have a hard time concentrating and sleeping. I am constantly worried and frightened that something wrong might happen to my family. I lose sleep and joy over the slightest word, action, or movement of my brother or my parents. Whatever they are doing, I find implication that they are hurting the family or damaging our family dynamic. While some patients express their suicidal ideation, I am the only one who knows that I plan it and think of it myself. Yet I am the one who tries hard to encourage everyone else. You know, Leo, I grew up not having ‘I’ as an identity. I learned early that everything and everyone in the family comes together as a whole and all of us are an inseparable ‘family’. Only in the past three-and-a-half years of working as a doctor have I realized that I am myself, and I am responsible for the things I do, the thoughts I have and the words I speak. While this is not necessarily a bad thing, it can be confusing, especially when I face the rebelliousness of my brother, who for so long I’ve felt responsible for. I need to separate myself from this somehow, but it is hard to break habits. Responsibility is such a big part of my nature. I also feel responsible for the quality of people’s lives at work, which is a huge source of stress. It weighs on me and the long hours and endless duties cause me to struggle and feel withdrawn sometimes. This has really manifested in the past few days. I can’t wait to go to bed and I dread the thought of a new day, because I’m overwhelmed and too busy. I feel ashamed for thinking these thoughts, and I also feel like a big failure because I have not been with my family, supporting them, cheering them on or earning enough money to sustain them.
